# How to add a lead capture form to a QR code

> To capture leads from a QR code, put a form between the scan and your page. Step by step in Linkbreakers: make the code, add a Contact Form, qualify the lead, say thanks, and find every lead in Visitors.

## Short answer

To capture leads from a QR code, put a form between the scan and your page. People scan with their phone's camera, leave their name and email, and continue to your page, while every lead shows up in your dashboard, live, with their answers and the code they came from. There is no app for them to download and no badge scanner to rent. In Linkbreakers, it takes four steps: make the QR code, add a **Contact Form** on the connector between the scan and the page, scan it once to test, and find your leads in **Visitors**.

## Quick summary

- The workflow becomes: scan, form, thank-you page, your page
- **Contact Form** comes with first name, last name and email, all required
- **Automatically skip if data is known** is on by default: someone who already left their details is not asked again
- One Dropdown question, such as "When do you plan to buy?", qualifies each lead
- Leads appear in **Live Events** as they arrive and in **Visitors** with name, email and answers
- One link per placement (booth sign, flyer) shows which placement brought each lead
- Forms and Visitors are on every plan, including Free

## Step-by-step

### 1. Make the QR code

1. In the [Linkbreakers dashboard](https://app.linkbreakers.com/workspace/dashboard/links), go to **Links** and click **Add new link**
1. Paste the page people should land on, and click **Next**
1. Give the link a name that says where the code will be printed, for example "Booth sign", and click **Create**

That is your QR code. Right now, its workflow sends a scan straight to your page.

### 2. Add the form

1. On the workflow canvas, hover the connector between the scan and your page, and click the plus
1. Choose **Form**, then **Contact Form**. Its fields are ready: first name, last name and email, all required
1. Check the two skip settings:
   - **Automatically skip if data is known** is on. If the visitor already has the required data on file, the form is skipped, so someone who already left their details is not asked again
   - **Allow visitors to skip this form** is off. Tick it to make the form optional: a skip button lets visitors continue without answering

### 3. Qualify the lead

1. Click **Manage Fields**, then **Add Field**
1. Label it with your qualifying question, for example "When do you plan to buy?"
1. Make it a **Dropdown**, give it a key (for example `timeline`), and add the options (for example Now, This year, Just looking)
1. Click **Done**, then **Insert Form Step**

Now every scan meets the form before your page.

### 4. Say thanks

1. Click the plus on the connector after the form, and choose **Message Page**
1. Write your thank-you as the title and description, for example "Thanks! We'll be in touch."
1. Keep the **Button** navigation: its button takes visitors on to your page

Details on the options are in [How to create a message page](https://linkbreakers.com/help/article/how-to-create-a-message-page).

### 5. Scan it with a phone

Open **Live Events** and scan the code. The scan shows up right away, and the form opens on the phone. Once the form is submitted, the row fills in with the visitor's email, without a refresh. Open the row to see the "Form Submitted" event with every answer.

## Find every lead in Visitors

Every lead also lands in **Visitors**, with a name and an email. Open one to see:

- the qualifying answer, under **Custom Attributes** (saved under the key you gave the field)
- the link it came from, under the links the visitor visited

Three tools turn that list into a follow-up list:

| What you want | How |
|---|---|
| The leads from one placement | **Filter by link** |
| Only the hottest leads | **Filter by attribute**, for example everyone who answered Now |
| A file for your CRM or email tool | **Export CSV**, which downloads the filtered list with every name and email |

### One link per placement

Putting the same form on a flyer? Give the flyer its own link, with the same form. Then **Filter by link** in Visitors shows which placement brought each lead. A single QR code reused on the booth sign and the flyer cannot tell them apart. For a booth with several placements (table tent, banner, demo screen), see [How trade show exhibitors use QR codes to capture and qualify leads](https://linkbreakers.com/help/article/how-trade-show-exhibitors-use-qr-codes-to-capture-and-qualify-leads). ## Limits and caveats

- **Export CSV leaves the custom answers out.** When we downloaded it in October 2026, the file had every name and email, but the qualifying answers came out empty. Use **Filter by attribute** before exporting, read answers on each visitor's page, or receive each submission with its answers through a [webhook](https://linkbreakers.com/help/article/how-do-webhooks-work-in-linkbreakers).
- **No native CRM sync.** There is no built-in HubSpot or Salesforce connection; a webhook, directly or through Zapier, Make or n8n, sends each submission on.
- **The form opens in the phone's browser.** The visitor needs a data connection; a weak signal in a large hall slows the page down.
- **Returning visitors skip the form** while **Automatically skip if data is known** is on. Untick it if every scan must show the form.
- **Fields have no conditional display.** To act on an answer, add a [visitor data condition](https://linkbreakers.com/help/article/how-to-use-visitor-data-conditions) after the form.
- **Personal data comes with legal obligations** that depend on where you and your visitors are. Say why you collect it, and link your privacy policy in the form description.

## Frequently asked questions

### Do visitors need an app to fill in the form?

No. They scan with the phone's camera and the form opens as a web page. Nothing is installed.

### Is a lead capture form available on the free plan?

Yes. Forms and Visitors are on every Linkbreakers plan, including Free, which costs $0 and needs no credit card ([pricing](https://linkbreakers.com/pricing)).

### Can I change the form after the QR code is printed?

Yes. The QR code points to its short link, and the workflow behind it can change at any time: add a question, remove the form, or change the page, without reprinting.

### Which form type should I start from?

**Contact Form** for leads (first name, last name, email). **Inquiry Form** collects an email and a message, **Rating Form** a score and a comment, and **Blank Form** starts empty. All four can be edited with **Manage Fields**; see [How to create a form](https://linkbreakers.com/help/article/how-to-create-a-form).

### How do I see how many people who scanned filled in the form?

Ask the [Assistant](https://linkbreakers.com/help/article/the-assistant-section) for the link's step funnel, or call `POST /v1/analytics/funnel` in the API. It counts the visits that reached each step of the workflow and how many went on.

### Can visitors skip the form?

Only if you tick **Allow visitors to skip this form**. Otherwise a visitor who does not submit stays on the form and does not reach your page; the scan is still recorded.

### Can I send each lead to my CRM as it arrives?

Yes, with a [webhook](https://linkbreakers.com/help/article/how-do-webhooks-work-in-linkbreakers). It posts each form submission, with the answers and the link, to your endpoint, and can be limited to one link. The journey from scan to form to page is covered in [How to build a customer journey with forms](https://linkbreakers.com/help/article/how-to-build-a-customer-journey-with-forms).
